@Power2k The important component to this entire thesis is that you can't compare apples to oranges here. Data has shown that duration of trial has an effect on results. Additionally, there are many factors that would allow for data anomalies. The only way to compare $ALT and anyone else is to have ALT follow any and every trial to the exact specifications (no various) and then compare results. Unless you do that, it's comparing apples to oranges.
With regards to reducing body weight, are you referring the weight loss results from the Obesity trials or the MASH trials?
Also, do you have the link to the Resmetirom data published by $MDGL? I just want to make sure I'm looking at the same thing.

2. Manufacturing / facilities is in the spotlight
As stated in the earnings call, they have a facilities inspection slated for Second Quarter of 2025 - this is a major hurdle they need to clear and by all accounts they seem well prepared. I had touched on this topic back in Feb -> https://stocktwits.com/LineBacker04/message/605724536. The update is that they're now a GMP Certified facility and they have plans to scale up one clean room module at a time until they reach their goal. They can currently support 250-odd patients, which will scale up to 3,000+ patients by Summer 2026.
3. Clues on Future Revenue
They're expecting $80M Milestone payment on Approval + Priority review voucher - this is something that will kick off at the outset (if they're approved) in Q3. In the call they also mentioned 50-60% DMD patients eligible for treatment with positive response from the payors. That puts us at 7,500 patients with approx. 40% revenue share, with the EU licensing still not fully closed. |

"All quiet on the Western Front"
I've tried to think of an analogy for our current situation (with regards to progress and valuation) and after considering options like "Calm before the storm" or "Light at the end of the Tunnel", I settled on the title of Erich Maria Remarque's classic World War-I novel. This is NOT a quiet period for $CAPR - there is rapid progress along multiple fronts - but you wouldn't know from looking at the "All quiet" nature of our valuation.
That being said, here are some updates with added color.
1. Still no news on the Ad-Com
I'm on record saying that there won't likely be an Ad-Com, but they are prepared for it (with a parade of patients, advocates, clinical experts, statisticians, etc. all lined up). If it does come to that, it would be another moment to shine in a public spotlight. The key areas where the FDA raises questions are usually:
a) Safety Profile
b) Statistical significance of data
c) Small Sample Size
d) Clinical as well as Statistical benefit |

4. "Multiple Treatment Effect"
This might have been missed by some, but its significance cannot be overstated. Deramiocel has demonstrated Multiple treatment effect - improvement in *BOTH* Cardiac and Skeletal function with both Clinical and Statistically relevant modulation. This is an unmet need with no approved medication having the same effect. This has also allowed them to demonstrate a clearly defined mode of action.
5. Miscellaneous points
Confirmed that NS has a Salesforce of 125+ personnel lined up
Confirmed dosing in 250+ patients till date
Confirmed that HOPE-3 is not relevant to current submission
Confirmed Becker's update in Q2/Q3
Confirmed BLA is for full approval and not accelerated
The phrase "All quiet on the Western Front" has come to symbolize stagnation, or lack of VISIBLE change, but there's plenty going on beneath the surface if you're paying attention.

Deramiocel basically checks off all the boxes - Stellar safety profile with 750+ doses administered, Statistically significant and clinically relevant effect confirmed through MRI, etc. To add more color, $CAPR used the RMAT designation to facilitate about nine (yes, nine!) type-B meetings with the FDA specifically to discuss the statistical significance of data. The "Natural disease progression" referred to in today's call was confirmed by prior data as well as the "GAP" phase in the OLE trial where the participants were off treatment. I've seen people comment on LVEF being only "Slightly better" at 36 months - are you kidding me? For a disease where LVEF degrades constantly in the untreated population this is a HUGE outcome and the FDA knows it.
That being said, if it came to an Ad-Com vote, I'd expect a unanimous or near-unanimous vote in favor of approval. Look out for the questions if they announce a late Ad-Com! |
